Request A Special Event

Our goal is to create healthier communities, and that means bringing education and services to people where they live, work and play. We hold workshops and screening events at churches, after school programs and senior housing.

If you'd like to have us partner with you on an event or have the Family Van to your community for health screening and consultation, please submit our event request form.  

Be sure to check the Van Schedule to see if your date is available before submitting your request.
